Terms and Acronyms

Accountable Care Organization (ACO)

Allows providers to create ACOs  and offers incentives for health care providers to work together to treat an individual patient across care settings – including doctor’s offices, hospitals, and long-term care facilities. 2010

Electronic Health Record (EHR)

An electronic record of patient health information generated and shared by one or more encounters in any care delivery setting

Electronic Medical Record (EMR)

The legal record created for a patient, client, or resident in a health care environments that is the source of data for the electronic health record

Health Information Exchange (HIE)

The electronic movement of health-related information between organizations according to nationally recognized standards

Health Information Technology Economic and Clinical Health Act (HITECH)

Provides HHS with the authority to establish programs to improve health care quality, safety, and efficiency through the promotion of health information technology (HIT), including electronic health records and private and secure electronic health information exchange

Healthcare Portal

Health provider networks that deliver consolidated access to patient information through a single entry point

The Health Insurance Portability and Accountability Act of 1996 (HIPAA) Privacy and Security Rules

The HIPAA Privacy Rule provides federal protections for personal health information held by covered entities and gives patients an array of rights with respect to that information. At the same time, the Privacy Rule is balanced so that it permits the disclosure of personal health information needed for patient care and other important purposes

The Security Rule specifies a series of administrative, physical, and technical safeguards for covered entities to use to assure the confidentiality, integrity, and availability of electronic protected health information.

Meaningful Use

By January 1, 2015, the voluntary objectives that providers must meet to qualify for the bonus payments and the technical capabilities required for certified EHR technology

Minnesota Interoperable EHR Mandate

All healthcare providers and hospitals have interoperable EHRs by 2015
[Minnesota Statute 62J.495]

Minnesota e-Prescribing Mandate

All providers, group purchasers, prescribers, and dispensers establish and maintain an electronic prescription drug program that complies with applicable standards by January 2011.[Minnesota Statute 62J.497]

Standards & Certification Criteria Final Rule

This Final Rule represents the first step in an incremental approach to adopting standards, implementation specifications, and certification criteria to enhance the interoperability, functionality, utility, and security of health IT and to support its meaningful use

TeleHealth

The use of electronic information and telecommunications technologies to support long-distance clinical health care, patient and professional health-related education, public health and health administration. Technologies include video-conferencing, the internet, store-and-forward imaging, streaming media, and terrestrial and wireless communications

Health IT - ONC

Health information technology (health IT) makes it possible for health care providers to better manage patient care through secure use and sharing of health information. Health IT includes the use of electronic health records (EHRs) instead of paper medical records to maintain people's health information. Certification Commission for Health Information Technology

Health IT companies wishing to certify a Long Term and Post Acute Care (LTPAC) EHR in our CCHIT Certified 2011 certification program should review the materials and follow the certification steps outlined below. Companies certifying EHRs in this "core" program may also choose the optional add-on Skilled Nursing Facility (SNF) and Home Health EHR certification programs.

Currently CCHIT has the only certification process for long term care providers.

Minnesota's 2015 Interoperable Electronic Health Record Mandate

This model describes seven steps that can be categorized under Adopt, Utilize and Exchange:
Adopt (Assess,Plan and Select)
Utilize (Implement and Effective Use)
Exchange (Readiness for Exchange and Interoperability)

CMS Overview of Meaningful Use

The Medicare and Medicaid EHR Incentive Programs provide a financial incentive for the "meaningful use" of certified EHR technology to achieve health and efficiency goals. By putting into action and meaningfully using an EHR system, providers will reap benefits beyond financial incentives–such as reduction in errors, availability of records and data, reminders and alerts, clinical decision support, and e-prescribing/refill automation. Spectrum Equity Investors and Trident Capital Announce Acquisition of HealthMEDX (Dec. 14, 2011)
Spectrum Equity Investors and Trident Capital today announced they have made an investment in the business of HealthMEDX, Inc, the leading provider of long term and post-acute care (LTPAC) technology, via an acquisition of the HealthMEDX, Inc. business. They will own a majority interest in the new company, HealthMEDX, LLC.


DHS to Host Listening Sessions on Pilot Project in January 10, 12, & 18, 2012
2011 legislation requires DHS to provide recommendations on how to develop a pilot project demonstrating a new approach to nursing facility care -- modeling a new way of providing care that is at a level between nursing facility care and assisted living.


Drug Safety Loophole Persists (Star Tribune) - Nursing homes are exempt from law requiring e-prescription use. Hospitals and clinics are slow to meet state requirement.


EHRs Certified For Long-Term, Post-Acute Care - These EHRs can't be certified for Meaningful Use, but interoperability with hospital systems is viewed as a key advance.

The Fourth Annual Long-Term Care Financing Solutions Forum will be held on Thursday, January 12 from 12:30 to 5 p.m. at the Hubert H. Humphrey School for Public Affairs. This free public event will focus on state and federal solutions to financing long-term care needs.
